Job Description

Summary:Provide clinical planning and intervention for partial hospital program clients. Consult with clinical team, educational staff, child's home school and family regarding treatment interventions and progress. Develop individualized treatment plans for clients according to state, CBH and partial hospital guidelines in Avatar RHD's Electronic health Records. Provide and participate in daily, structured, milieu programming for partial hospital clients and facilitate psycho­ educational/counseling/family support groups as indicated. Attend discharge/transition meetings with family, child's home school and other team members as needed. Attend all supervision and training as required by the State, agency guidelines and other relevant continuing education as outlined by Program Director. Shift:  08:30 AM - 03:30 PM [Lunch: 12:00 PM - 12:30 PM] 5 days a weekPay: $26.00/hour Essential Duties and Functions: 
  1. Assess behavioral needs and formulate treatment goals of client in conjunction with client, other partial staff members, teacher, and family members.
  2. Formulate, develop and implement individualized treatment plans for clients in
  3. Avatar (EHR) and in conjunction with the treatment team, according to state and CBH guidelines.
  4. Maintain a caseload of assigned clients and perform all administrative tasks associated with this caseload, including, but not limited to: documentation of progress notes, treatment planning, case conferences, discharge and aftercare
  5. planning and case management tasks in Avatar (EHR).
  6. Provide and participate in daily, structured, milieu programming for partial hospital clients under supervision of the program Director
  7. Conduct individual group, and family therapy as indicated.
  8. Facilitate psycho-educational/counseling/family support groups as indicated.
  9. Consult with teacher and assist in the development of behavioral plans for classroom intervention as needed.
  10. Provide emergency clinical/crisis services as necessary.
  11. Documentation of all services provided to the client, according to prescribed timelines in Avatar (EHR).
  12. Monitor progress of treatment plan in Avatar and collect relevant data on an ongoing basis.
  13. Provide ongoing consultation with treatment team, classroom teacher and family on clinical issues as indicated.
  14. Monitor and assist in the coordination of services for client/family.
  15. Provide communication and clinical consultation to client home school to facilitate discharge and successful transitioning.
  16. To maintain and attend regular and appropriate supervision with the program Director and/or their designee on an at least weekly basis.
  17. Attend team meeting with all relevant team members:
  18. To complete 20 hours of annual continuing education, if unlicensed.
  19. To complete the minimum number of continuing education credits necessary to maintain any licenses and certifications, as applicable.
  20. To adhere to assigned working hours.
  21. Be available to work one evening per week, with prior notice.
  22. To perform other duties as assigned.
    Qualifications:
  1. Must show evidence of a Master's degree in psychology, social work, education, criminal justice, counseling or a related human service field along with two years of experience working with children.
  2. Must show evidence of licensure where licensure is available.
  3. Must complete a CBH credentialing application.
  4. Must provide two (2) professional references prior to any direct contact with children.
  5. Must agree to complete·a cl irninal history check and child abuse clearance upon hire and every two (2) years thereafter.
  6. Must agree to complete an FBI criminal clearance, as applicable.
  7. Must complete an orientation to both the partial hospitalization program and Resources for Human Development within the first 90 days of hire.
  8. Must complete all CBH and state mandated training within 90 days of hire and annually thereafter.
  Required: 
  1. Master’s degree in related field
  2. Minimum 2 years related experience
  3. All three clearances-Child Abuse, FBI, PA State Diploma Original transcript Degree verification
