Salary: $25.11 | PHAbout the Role:
We are seeking a dedicated and compassionate Certified Addictions Counselor (CAC) to join our progressive, interdisciplinary team. In this role, you will play a vital part in supporting individuals, families, and the community by providing comprehensive addiction counseling services. Your work will contribute to health promotion, disease prevention, and chronic disease management, ensuring patients achieve their highest level of wellness and recovery.As a valued member of our primary care team, you will utilize your expertise to counsel and educate patients and their families, guiding them through their recovery journey.Key Responsibilities:
  • Collaborate with various departments, including Primary Care, Integrated Behavioral Health, and Outreach teams, to provide comprehensive care coordination for Medication-Assisted Treatment (MAT) program patients.
  • Conduct comprehensive patient assessments, new patient intakes, and develop personalized treatment plans with clear objectives and outcome goals.
  • Provide individual and group counseling using evidence-based approaches such as Trauma-Informed Care, Motivational Interviewing, Harm Reduction, and 12-step facilitation.
  • Educate patients on Suboxone induction, naloxone administration, and relapse prevention strategies.
  • Facilitate peer support groups and provide ongoing support in areas such as daily living, interpersonal skills, and community integration.
  • Coordinate referrals to detox, inpatient, and residential treatment facilities while assisting patients in navigating healthcare resources.
  • Regularly monitor patient progress, provide follow-ups, and support transitions between different levels of care as needed.
  • Participate in team meetings, quality improvement initiatives, and ensure accurate documentation within Electronic Medical Records (EMR) systems.
  • Manage patient schedules and coordinate with administrative staff to ensure smooth clinical workflow.
  • Participate in after-hours on-call rotations to provide 24/7 patient access to services.
What We're Looking For:
  • Education: Bachelor's degree required.
  • Certification: Current CAC certification from the Commonwealth of Pennsylvania.
  • Experience: Minimum of 2 years in a drug and alcohol counseling setting; outpatient experience preferred.
  • Training: Completion of an 8-hour Buprenorphine training program (can be completed post-hire).
Key Skills and Competencies:
  • Strong knowledge of primary healthcare, addiction recovery, and social determinants of health.
  • Proficiency in health promotion, disease prevention, and behavior change counseling techniques.
  • Excellent organizational and time-management skills to handle multiple tasks efficiently.
  • Experience with Electronic Medical Records (EMR) and willingness to adapt to new systems.
  • Ability to build trust and maintain professional relationships with patients and colleagues.
  • Commitment to providing trauma-informed care and fostering a supportive environment.
  • Strong problem-solving, communication, and conflict-resolution abilities.
